What is a Continuum of Care in Mental Health Treatment?

At Sanford Behavioral Health, we provide comprehensive and personalized treatment for individuals diagnosed with addiction, eating disorders, and mental health conditions. We utilize a “continuum of care,” which means giving support at every level of recovery, from intensive inpatient treatment to virtual therapy. All of our programs are entry points to the recovery continuum of care. When individuals are comfortable within the organization, it makes for a smooth transition from one level of treatment to the next. A person steps up or down through levels of care.

What is a Continuum of Care?

Sanford West Behavioral Health Campus provides comprehensive treatment for a wide range of mental health diagnoses, addressing the specific and individualized needs of each client. Our integrated approach includes access to psychiatry providers, nurses, clinical therapists, recreational therapists, case managers, dieticians, and more, all under one roof.
We recognize that successful treatment of mental health disorders requires ongoing evaluation and adjustments. Consequently, we place each individual in the most clinically appropriate and least restrictive level of care. Furthermore, we understand the significance of a long-term continuum of treatment programs, classes, and resources. This is why we actively engage with our clients to ensure a seamless transition into a safe, long-term environment after residential treatment. Alternatively, If someone enters our care at the outpatient or virtual level, they can “step up” to higher levels of care if needed. We also provide a supportive and structured aftercare program to promote sustained wellness and long-term recovery.

The Continuum of Care

“The huge benefit to becoming a Sanford client is that we can assess and transition an individual during all stages of treatment within the same building, the same location, and with the same psychiatrist and clinical team. Consequently, once someone is part of the Sanford continuum of care, no matter what transpires as they go through their lifetime of recovery, they have a powerful alliance.” Sanford Executive Vice President & Chief Operating Officer, Katie Vokesย
